Habilitation Services

Services to help people learn life skills.

What is Habilitation Services?

Services offer a variety of interventions aimed at improving the functioning of people with developmental disabilities. Among the services available are habilitative therapies, special developmental skills, behavior intervention, and sensorimotor development.

What We Provide

Life Skills Development

Training and practice in fundamental skills like shopping, banking, money management, and community survival skills.

Social Skills
 

Support in developing friendships, assertiveness, and problem-solving abilities for use in daily interactions.

Therapeutic Support

Implementation of therapeutic recommendations including speech, occupational, and physical therapy.

Service Objectives

To provide a wide range of support services to promote the consumer's physical, emotional, and mental well-being.

To allow the consumer to gain knowledge based on his/her choices, and participate in his/her community.

To provide training and supervision to the consumer in order for him or her to improve or maintain his or her self-help, socialization, and adaptive skills.

To cultivate positive relationships with others.

5

To provide opportunities for people in the community to interact with one another.

To help the consumer achieve and maintain a quality of life that promotes the consumer's future vision and priorities.

To ensure all residents' health and safety.

Service Requirements and Limitations

This service shall be provided to residential consumers with a variety of needs, including behavioral, physical, and medical challenges. These settings typically serve two (2) to four (4) people, but they can accommodate up to six (6) people.

When the consumer is hospitalized, this service is not provided.

All Group Homes, NestVillage shall have applied for and been awarded the service of Room and Board.

All Group Homes, and shall provide Room and Board at all relevant times.

All Group Homes. This service has been approved for the day.

NestVillage is not relieved of its obligation to continue serving a consumer when that consumer's needs change. When the consumer's needs change, NestVillage shall send written notice of the change to the consumer's Support Coordinator as soon as possible (within one business day). NestVillage will begin a collaborative planning process with the consumer's planning team in order to update and revise the consumer's planning document. When a medical change is required, the staff at Meaningful Life Behavioral Health will be involved.

Transportation to and from Meaningful Life Behavioral Health will be in charge of schools, day programs, and employment services. NestVillage is also responsible for other reasonable transportation within the community, including transportation fees. If a consumer chooses not to use the group home-provided transportation and the consumer's choice to use public transportation is documented in the consumer's planning document, the consumer is responsible for any fees associated with the public transportation.

NestVillage must comply with Arizona Administrative Code ("A.A.C.") Title 6, Chapter 6, Article 8.

Goals of Service

The following goals must be met by NestVillage LLC:

Assist in the development of: in accordance with each consumer's planning document (e.g., ISP).

Outcomes related to habilitation that are based on assessment data and input from the consumer and the consumer's representative will enable the consumer to achieve his or her long-term vision and priorities.

Within twenty (20) business days of the start of the service for a new or continuing placement, and whenever a new outcome is identified for the consumer, a specific teaching strategy for each habilitative outcome is developed. The specific teaching strategy for each outcome must specify the implementation schedule, frequency of services, data collection methods, and steps to be taken to teach the new skill.

Changes to specific outcome(s) and/or strategies agreed upon by the consumer's planning team based on the consumer's presence or absence of measurable progress.

As identified in each consumer's planning document, offer a wide range of support services such as assistance and referrals concerning personal and physical requirements, as well as routine daily living skills.

Implementing strategies to address DDD's behavioral concerns, developing behavior support programs, and coordinating with behavioral health programs to ensure medication treatment plans are properly reviewed.

Assuring that the consumer's health needs are met, including providing follow-up as directed by the consumer's Primary Care Provider ("PCP") or medical specialist.

Implementing all therapeutic recommendations, including speech, occupational, and physical therapy, as well as assisting customers in adhering to diets, exercise routines, or other therapeutic programs.

Mobility training, alternative or adaptive communication training; general supervision; and opportunities for training and/or practice in fundamental life skills such as shopping, banking, money management, access to and use of community resources, and community survival skills.

Develop, maintain, or improve self-sufficiency. Sensorimotor skills, cognition, personal grooming, hygiene, dressing, eating, toileting, self-medication and first aid, recognizing symptoms of illness, and preventing accidents and illnesses are all important for each consumer. Basic hygiene, grooming, and first aid supplies must be available in order to fulfill this mandate.

Assist each consumer in developing methods for initiating and maintaining friendships of his or her choice, as well as appropriate assertiveness, social skills, and problem-solving abilities for use in daily interactions.

Provide opportunities for consumers to participate in community activities and facilitate the use of community resources.

Arrange and plan transportation to assist each consumer in all daily living activities (e.g., day treatment and training, employment situations, medical appointments, visits with family and friends, and other community activities). Encourage, as appropriate, the acquisition of abilities required to access community transportation resources.

Create a monthly on-site/community-integrated schedule of daily activities and document consumer input directly into the schedule. Consumer preferences, developmental level, planning document goals, and enrichment of life experiences all influence daily activities and schedules. Allow for a reasonable range of activity participation options and provide alternatives. This schedule must be made available upon request to consumers, consumer representatives, and others.

Assist in ensuring that services are complementary with other involved entities, such as day treatment and training providers, health care providers, and schools, and that they are coordinated to meet the needs of the consumers served.
